Personal information necessary for provision of medical services is information that is collected according to relevant regulations, such as the Medical Service Act, regardless of patient’s will. 1) Purpose of Collection and Use ① Identity verification for consultation/examination/appointment and provision of medical services, including diagnosis and treatment ② Charging & billing of medical fees, issuance & mailing of receipts and certificates 2) Collected information : Patient information(name, resident registration number, contact information, address), medical information 3) Retention period : Retained until the point of attainment of its purpose (including mandatory period prescribed by the Medical Service Act and relevant regulations)
2. Collection & Use of Personal Information for Provision of Extended Services Agree
1) Purpose of Collection and Use ① To provide hospital administration services, such as appointment notification service ② To provide information related to education, seminars, health information, and clinical research to patients ③ To provide CS services, including counseling 2) Collected information : Patient information(name, contact information, address) 3) Retention period : Retained until the point of attainment of its purpose (including mandatory period prescribed by the Medical Service Act and relevant regulations)
